The last time I got a commitment from our CEO was to go to a team meeting for presentation of the new business plan (He arranged this meeting - so he was there) then to ask him afterwards "have you got a couple of minutes?" It worked only because he wasn't off to another meeting and it was late in the afternoon.
In the meeting, the CEO was often absent - "just printing out this" "just finding an email". It was very frustrating from the team perspective, and as we were asking some potential team members to attend it was a little embarrassing - did not look good - the CEO going in and out of the meeting he was chairing.
The couple of minutes eventually was about 1/2 hour long and we discussed many of my concerns. It helped that I had created a wiki, had organised my facts, ideas and provided examples of where things went wrong, where they were OK and what we could / should do. IN a nutshell he was presented with problems and solutions.
Agreement with my plans (from a training perspective, not a business growth perspective). But no action on some technicalities until I went behind his back and spoke with the chief developer - who went ahead and just did stuff!
